Hello everyone and welcome to this new episode of the "1/4 H Neuro" podcast. Today, we will discuss a topic that affects many people: shoulder issues. I will share a case study of an athlete who consulted for shoulder pain.
I primarily work online, which involves conducting an initial history questionnaire. This allows me to create an accurate assessment of movement capabilities and limitations in the targeted joint. The athlete in question had experience, being both a physiotherapist and a high-level athlete, which facilitated our communication.
The assessment revealed reflex stabilization deficits, especially on the side contralateral to the painful shoulder. Whenever he raised his arm, he felt tensions that limited his mobility and strength. This issue was present both with load and during free movement.
Interestingly, even though the pain was localized to the right shoulder, the reflex stabilization problems were more pronounced on the left side. This led us to understand that reflex stabilization must take precedence over voluntary movement. Thus, voluntary movement is broken down into three stages: anticipatory reflex stabilization, voluntary movement, and retroactive reflex stabilization.
The training plan focuses on axial stabilization before addressing other aspects. Here are the main areas:
The first key area of focus is on breathing mechanics. A breathing exercise has been introduced to stabilize the central axis, which has a direct impact on reflex stabilization.
We then worked on the three main components that govern human functioning: vision, balance, and movement. Each intervention must therefore encompass these three fundamental elements.
An interesting exercise I proposed is the "infinity walk." This exercise encourages cooperation between sensory systems while integrating stabilization principles.
This training program is designed to be carried out for at least a week, with longitudinal follow-up to adapt the program according to the needs of the nervous system.
